A charge of $20 per surname is charged for the Index service + $2 postage. We will accept unused postage stamps. Photographs are $6 each. Transcriptions if sent by email free or $3, Stamped self addressed envelope is requested for photos. Please make sure you specify Surname and Given name for cemetery photographs. This form can be filled out, printed, posted with remittance for Index surnames, Cemetery Transcriptions or Cemetery Photographs.
